Every state I have been involved with requires lesson plan, maintaining accurate attendance, and keeping a gradebook.  I have seen elaborate, multiple-page lesson plans to a lesson plan book with a little box for each class to record briefly what the teacher plans to do each period of the day.  We have taken the standard-based classroom requirements and created a lesson plan template that streamlines the planning process, we created a lesson plan template for special education co-teachers to document planning and services, and we have worked with teachers to monitor the process to teach and reinforce the benefits of collaborative, data-based, standards-driven  lesson planning. Although this can go hand-in-hand with collaborative planning, lesson planning is the first hurdle to overcome with teachers who consider them as personal or who try to recycle them from year to year. We can provide support from feedback on current practices to completely creating a program to meet your specific needs.
